Here we will attempt to address our policies as far as fees and insurance and other methods of payment are concerned. Please be aware that prices can vary from time to time due to reasons beyond our control!

Now here is an important question that patient need to answer to themselves. Why invest on your health? It doesn't matter what type of visit you take part in. Investing in your health is just that – “an investment”. Unlike other investment such as material or financial investment, health investments should provide you with gained energy, strength, and stability, among many other benefits. Ask yourself what good are material things for without the one true gift of good health and feeling good about yourself?

A.-WHAT WE WILL ATTEMPT TO PROVIDE TO OUR PATIENTS IN OUR OFFICE!

At Dr.Dieguez’s  office, we will strive to achieve that ultimate goal for you- that goal of being healthy and active without pain and feeling good about yourself. However only you can decide what is fair and “worth the investment” when it comes to out of pocket cost for your healthcare especially when it comes to high end therapies like Prolotherapy and Orthobiologic therapies such as Platelet Rich Plasma (PRP), BMAC therapy, and Progenekine. We will strive to instill in you the feeling of “that was worth it” when you come to our office to see Dr. Dieguez for your care.

At our office all prices are explained up front and neither treatments nor lab-work, therapy, or any other intervention are initiated or ordered without the patient’s prior approval.  

 We respectfully request that patients pay in full the portion they are responsible for at the time of service. Our office accepts all major credit cards and we also accept cash (or any combination of the above). If we are participating providers with your insurance company and the services that we provided to you is a covered service, we will bill your insurance company and you will only be responsible for any copay, co-insurance or deductible you may have.

B.-THE FOUR GROUPS OF PATIENTS WE ENCOUNTER NEEDING SERVICE

At our office we are very aware of the problems some patients are having with their health care needs and we have tried to come up with ways to accommodate all of them so that they can at least get the basic care they deserve. Having insurance coverage does not always means having access to care. In fact, it may become obstacle at times! Our philosophy is to do our very best to accommodate everyone.

To better visualize the issues, we can divide the patients we see in an office visit as belonging to four big different groups

 1. Patient with insurances we participate with:

a) For services covered by your insurance plan, you pay only your deductible if not met yet and any copays and coinsurance. We are required to collect this at time of your visit. Do you see the reason why? Insurance companies hope that by you having to pay this deductible money (sometimes as high as $5000/year), it will stop you from incurring in more expenses which the insurance is obligated to pay because of your contractual agreement with them.  We bill the rest to your insurance company and wait for them to pay us for what remain.

The insurance company’s goal is for you to spend nothing of what they have contractually agree to pay and to bill you every month for the “coverage” they provide! Smart, isn’t it? If you add what you pay in premium monthly plus the deductible, copays etc. you will realize that before your insurance pays a cent, you will pay many thousands of dollars. Many people pay over $11000 a year between deductibles and premium!! We have seen deductible as high as $17000 per year!

b) For noncovered services like PRP, Prolotherapy and others, you the patient pays it all out of pocket unfortunately but;

•  You may qualify for a payment plan

•   We offer concierge subscription plans.

 2. Patients with no insurances or insurances we do not participate with: These patients can take advantage of the following for options:

• Our specials available from time to time.

 3. Patients in a variety of circumstances: such as no insurance at all, patient with very high deductible insurances, patient wanting better care not provided by their insurance, patient facing delaying tactics and having to jump thru hoops like having to do physical therapy six weeks before getting the injection they need to relieve their pain. Or patients facing delaying tactics by their primary care doctor who runs an HMO and is incentivized (paid bonuses) not to provide referral to a specialist for the care that the patient needs to save money for the HMO. These patients at our office can take advantage of the following options:

• Our Direct Specialist Care Subscription plan

4. Patients with or without insurance wanting treatments not covered by insurance. Example of treatments not covered by insurance may include Prolotherapy, Platelet Rich Plasma (PRP), Fat transfer procedures like Progenekine, Bone Marrow Concentrate (BMC) injections, etc. This group of patients looking for these more advance and sophisticated treatments have the following options:

This cash rates that Dr. Dieguez offers are basically for patient that have no insurance coverage whatsoever or have an insurance we do not participate with so that they can have access to basic care at our office at a very affordable price. These prices are not for Orthobiologic procedures like PRP, Progenekine, cell therapies, or Prolotherapy.  Please keep in mind that some of our supplies have quadruple in price withing the last two year.

D. OUR DIRECT SPECIALIST CARE SUBSCRIPTION PROGRAM.

This program is for patients again with no insurance  or  insurances we do not participate with. No emergencies and no controlled substance prescription!  1.A brief Pre-enrollment “Consult” is required to assess if we can be of help to you. Only an evaluation will take place. No test will be ordered until you enroll. $100/patient

 2.Enrollment Fee. The day the enrollment agreement is signed: $75/per patient.

3. Monthly membership. For one person: $125. Spouse $50 extra. Family member 18 to 21years of age.  $40 each. Charged 30 days after enrollment. This covers three visit per month per patient from enrollment date. No copays. Any additional visit over three  is $40 each visit.

4. Additional fees. For any injection procedures there is an additional $35 charge the day of the injection. For viscous supplementation (Suppartz) procedures for the knee, the charge is $70 per injection on the day of injection. For bracing and orthotics, you pay $15 over cost.

 5. Orthobiogics and Prolotherapy procedures. These are discounted 50% from our

Standard rates for our subscription members, if the patient stays with the practivece for a 12 months period after the procedure. If the patient leaves the practice before, the credit card will be charged the full regular price of these procedures.

 6. Direct Cell phone access and texting with the doctor. Intended for simple personal

Issues like refill medications we prescribed, questions regarding medications or need for non-acute diagnostic testing. These may come in handy, and provide expert advice.

E. OUR “CONCIERGE” SPECIALIST CARE SUBSCRIPTION PROGRAM.

This program is for patient either with insurances we participate or not participate with, that are wanting high end treatments not covered by any insurance companies. Specifically, Prolotherapy, Platelet Rich Plasma, Progenekine and Cell Therapy procedures such as bone marrow concentrate (BMC). Non-emergecies only! No control substance prescription. For stablished patient only! Here are your choices:

a) Direct Cell Phone Access to Doctor. Calls intended to resolve simple personal issues like refill medications we prescribed, questions regarding medications or need for nonacute diagnostic testing. These services may come in very handy at the time for expert advice! How many times have you wished you could ask a question to your doctor? A secure app will need to be used. Unlimited calls per month. Yearly payment and agreement required. $ 300.00 /year.

b) Texting with the Doctor. Texting may be all you need for simple questions. Chatting   will be done via text messaging using a secure app. Unlimited text. Yearly agreement required. $ 192.00/year

c) Gold package. Includes all of the above plus: a) blood test and imaging orders due to personal or work-related reasons, but that do not need a medical consultation with our physician.Copies of test will be provided as well as any recommendations if needed. b) Two Prolotherapy treatment a year if indicated OR One Platelet Rich Plasma Treatment (PRP) per year if indicated.c) Same or next day no wait appointment during normal business hours. G. ABOUT OUR FEE GUIDELINES

The above list has been provided as a guide for determining expected cost for various specialized services we provide. The actual price may be more or less depending on the exact areas being treated and what treatment you received. For example, two areas may need to be treated. At your New Patient Consultation Visit all the alternatives and costs will be explained to you and you will be able to ask and get answers to your questions.

H. NOTICE:

Please be aware that we are not one of those so called “pain clinic”! We treat the different musculoskeletal, orthopedic and spine conditions without pain medications. Having worked for the Florida Department of Health as an expert witness for the prosecution services unit reviewing pill mills type of operations, and being very aware of the addiction problem we have in our country, Dr. Dieguez rarely if ever prescribes narcotics and other controlled addictive medications. If he ever does, it is only if absolutely necessary after a minor surgical procedure and never for longer than three to five days. With that been said, if what you are looking for are narcotics or other controlled substances, don’t expect to get them in this office. Don't waste my time or yours. Or your money for that matter!This is not one of those places where you go and you exchange dollars for drugs. Here you will go out empty handed. And remember, we will not hesitate in reporting drug seekers to the authorities. We do not prescribe Marijuana or any of its derivatives either!
